Established in November 1999, Sunway Medical Centre is an Australian Council on Healthcare Standards (ACHS) and Malaysian Society for Quality in Health (MSQH) accredited private hospital.

Bone & Joint Centre

The Bone and Joint Centre at Sunway Medical Centre consists of a team of Orthopaedic Consultants that come together to provide patients with therapies and interventions to overcome their bone and joint problems. The Centre does bone and joint replacement surgeries, lumbar and spine surgeries as well as provide clinical care for other orthopaedic problems relating to rheumatic disease and sports-related injuries.

Total Knee Replacement

You can only walk without pain when the bones in your knee joint are smooth and cushioned by healthy smooth cartilage. You also need strong muscles and ligaments for stability because your knee is more than a simple hinge joint: each time you bend your leg to walk or climb stairs, the bone rotate, roll and glide on each other.

Knee pain and stiffness often result from osteoarthritis from injury or wear and tear. Other knee problems include rheumatoid arthritis (an inflammatory joint disease), previous infection, or poor alignment of your bones. With Osteoarthritis, your cartilage “cushion” wears away. Bones become rough and rub together, causing pain. With rheumatoid arthritis, your joint may also be inflamed and swollen.

A painful, stiff knee can keep you from doing the simple things in life, even walking without pain. Now your orthopaedic surgeon can replace your problem knee. After your knee replacement, you’ll have some restrictions on using your new knee, but you can look forward to returning to many of your activities of daily living.

MINIMAL INVASIVE UNI COMPARTMENTAL KNEE REPLACEMENT

The natural knee consists of three inter-connected compartments – the medial, lateral and patello-femoral (knee cap) compartments. Arthritis may affect one, two or all three compartments of the knee. The total knee replacement is today a highly reliable and effective operation for arthritis of the knee which involves two or three compartments of the knee joint. In this operation two or three compartments are resurfaced. Some knees are only affected in one compartment.

These patients do not require the full scale replacement operation. They can obtain very effective pain relief and return of good knee function by an operation that resurfaces only the diseased compartment.

The partial replacement of the knee join is known as Uni Compartmental Knee Replacement (UCR). This form of surgery has been available for almost as long as Total Knee Surgery had been performed. The early varieties of UCR were associated with variable outcome. Many patients obtained excellent results, but up to one in four patients who received a UCR in the seventies and eighties required a revision operation. Today, we have a better idea about patient selection for the operation. And there are better designs and better materials for the UCR. The modern UCR is capable of 98% survivorship after 10 years of implantation. Such a result comes close to matching the best outcome of the TKR.

The standard method of performing a UCR requires a full exposure of the knee joint. As such the operation is only slightly less major than a TKR. A newer method of performing a UCR is now possible.

The Minimal Invasive technique for UCR represents a major technical advancement in the art of joint replacement surgery. For the first time, surgeons are able to offer their patients a method of replacing the knee joint without the need for prolonged hospital stay, blood transfusion or a prolonged rehabilitation. The operation is performed through a relatively small incision and blood loss is usually under 60mls. Because of the operation is of a much smaller scale than a TKR, surgeons and patients are more comfortable with simultaneous surgery to both knees if the need arises. It takes 3 – 4 weeks for most patients to achieve full recovery. The surgery however demands more technical expertise to perform and the choice of prosthesis is an important factor for the success of the operation. A dedicated rehabilitation programme is necessary to achieve the best outcome.

Expected Outcome

There is quite considerable variation with the pace which patients “recover fully”. This is related to differences in age, physical fitness and a number of other factors. Most patients are quite comfortable and independently mobile after 4 weeks. Your new knee will serve you well for general, non-impact activities. You should avoid high impact activities such as jumping and running. You may participate in sports such as swimming, cycling, golf and bowls.

The prosthesis may loosen after many years of use. Most UCR are expected to last beyond 10 years. If the replacement fails conversion to a Total Knee Replacement is usually effective.

COMPUTER AIDED TOTAL JOINT REPLACEMENT

Ever since the first Charnley hip was implanted at the Wrightington Hospital for Joint Diseases in Wigan in 1961, millions of patients worldwide have benefited from total joint replacement. Although the basic principle has remained, there have been great strides in development in this field since Charnley’s time.

The main challenge today is to improve implant longevity and function and to this end there is a push for better material, improved design and improved surgical technique. One of the most recent innovations is computer aided total joint replacement.

Traditionally, preparation of the joint surfaces to accept the prosthesis is done by mechanical jigs and visual aids. Although this is acceptable in the majority cases, problems are encountered in obese patients, severe deformities and soft tissue abnormalities. One of the common causes for early prosthesis failure is misalignment of the new joint which is basically a technical error. It is envisaged that with computer assistance, this can be overcome.

Most suppliers in the joint replacement market will provide a computer assisted module for implantation of their products for the lower limb. Although the specifications may vary, the principle is the same i.e. to position the implant so as to re-establish the mechanical axis (hip-knee-ankle) of the lower limb.

They use GPS type technology for the surgeon-computer interface. Wireless trackers inserted in the patient relays information through infrared optics to the computer. The computer with its’ unique tracker software will in turn inform the surgeon continuously as to the position of the patient’s limb in space at any given time.

The procedure starts with registration of the patient’s anatomy. Unlike image guided surgery which requires pre-operative imaging (either MRI, CT or Fluoroscopy), computer aided surgery uses the information gathered by the surgeon during the operation to allow for the creation of a “morph” of the patient’s anatomy. The cutting jigs use trackers to communicate with the computer to ascertain that the saw cuts to be done will align the implant in the appropriate alignment to the accuracy of 0.5º.

Since the computer allows for real time display of the patient’s joint in space, one can ascertain the alignment and kinematics of the joint with the trials in place, allowing the surgeon to modify the implant position or balance the ligaments at the trial stage if necessary. This instantaneous feedback remains one of the major advantages of computed aided surgery over the traditional jig-based procedure where in the latter the surgeon can only ascertain the implant position from the post-operative X-ray.

There are other advantages. As the medullary canals are not breached (as in the traditional jigs), blood loss is less and therefore negating or minimising blood transfusion. This also leads to a lower incidence of fat embolism. As the alignment of the implant is optimal and with ability of the kinematics to be modified intra-operatively, post operative function is improved. Being an extra tool to “help us see”, there is also a great potential in doing joint replacement procedures through minimal incisions, again improving post-operative rehabilitation.

In prospective randomised studies 1,2 comparing computer assisted knee arthroplasty to conventional technique, the former has been shown to give significantly better results in terms of post-operative alignment and blood loss. We have performed computed aided total knee and hip replacement at the Sunway Medical Centre since October 2005. Comparing to a matched group of traditional jig-based total knee replacement done in the same centre, the computer aided group showed consistent result in terms of femoral, tibial and tibio-femoral alignment. Out-liers seen in the jig-based group were not detected in the computer aided group. Measurement using CT scannogram showed that the femoral, tibial and tibio-femoral mechanical axis together with the joint line coronal alignment all fall in the acceptable range of ± 3º. Out of these, 85%, 94%, 82% and 94% respectively fall in the ± 1º range.

We are encouraged by our initial results. It is envisaged that the computer will replace traditional methods in all total joint replacement in our centre and elsewhere in the future.

Some Interesting Facts
  • 1st private hospital to perform the total joint replacement surgery using computer navigation. To date, approximately 400 cases have been performed
  • Hip replacement, knee replacement and total joint replacements are performed on patients who suffer from pain and deformity
  • An implant can last between 15 – 20 years
  • Surgery done via computer navigation has proven to show accurate alignment in the patient. The patients’ legs look straighter, the implant lasts longer and the patient can also bend their knees a lot better
  • In younger people, using the minimally invasive uni-compartmental knee replacement technique, the patient has an incursion of only between 6-8cm. For these patients, their range of movement remains. They can even start driving within six weeks from the operationbetter
  • Patients are advised not to take any herbal medications as they tend to cause high bleeding
  • Joint replacement surgery does not prolong a patient’s life but it enables patient to carry out activities of normal living and enjoy a better quality of life
  • 95% of the patient population that undergo joint replacement surgery tend to be from the elderly group. The elderly population who live longer and have had an active lifestyle are more prone to undergo a joint replacement surgery. Those who have been involved in a lot of sports and have sustained many sports injuries are also likely to undergo a joint replacement surgery
  • 10 – 15% of patients with Rheumatoid arthritis also opt for joint replacement surgery
  • One can avoid having a joint replacement surgery if they seek professional help early when they suffer from any injuries, maintain an ideal body weight and avoid high impact exercises

Digestive Health Centre

Digestive problems happen to all of us from time to time—an upset stomach, indigestion, gas, heartburn, constipation, or diarrhea. These can be caused by certain types of food, food contaminated with harmful bacteria, or flu or other short-term illness. Many digestive problems are not serious and don’t last long and most can be eased if you exercise, reduce your stress and eat the right diet. Some of the most common digestive problems are:

The Digestive Health Centre provides a holistic care approach in dealing with conditions related to the digestive system that includes the oesophagus, stomach, small and large bowel, liver, pancreas and gall bladder.

  • Gastroscopy: An examination of the upper digestive system including the oesophagus, stomach and first part of the duodenum
  • Colonoscopy: An examination of the lower digestive system including the large bowel until the end of the small bowel called terminal ileum.

Good vision is something that we all want for everything – our daily activities, our adventure trip, our work and etc. Some people have it naturally while others have refractive errors such as nearsightedness, farsightedness and astigmatism.

Eyeglasses and contact lenses offer a good solution to those who have such an eye condition. However, some consider them a great inconvenience. They want to see clearly without any visual aids.

These days, we are blessed that the medical advancement makes it possible for us to gain back clear vision. With this being said, we are aiming on a long-term vision correction other than ordinary glasses or contact lenses – Laser Vision Correction (LVC).

This field has seen many developments within a span of 30 years. In particular, the three most widely performed techniques today (PRK, FemtoLASIK and SMILE) are able to treat most refractive errors very effectively and even gently. With the most advanced procedure – SMILE, it supports a comfortable patient experience with its innovative minimally invasive method.

Children

It is recommended that all children undergo a routine vision screening when they enter preschool. Children also need to consult an eye specialist without delay if any of the following signs are noticed:

  • Any unusual appearance of the eye
  • Holding objects very close to the eye
  • Sitting very close to the television or computer
  • Squinting, closing or covering one eye
  • One of both eyes turning in or out
  • Difficulty seeing the blackboard in the classroom
  • Eye redness, irritation, rubbing or watering

VISION SCREENING FOR CHILDREN

Some eye conditions in childhood can lead to a lifetime of weak vision (amblyopia, “lazy eye”). As young children are unable to complain about their eyesight, parents may be unaware that there is a problem until the time for effective treatment has passed. This is why all children should have a vision screening test as part of the monitoring of their development.

Our optometrists will carry out the vision screening and let you know the results. If they find an abnormality, they will advise you to bring your child to see an eye specialist for a detailed examination.

This test does not include:

  • Consultation with an eye specialist
  • Eye tests that need pupil dilation
  • Eye medications (if needed)
  • Additional eye tests or procedures

Adult

Symptoms that need attention include:

  • Blurring or loss of vision
  • Distortion in the vision
  • Blocked vision
  • Seeing unexplained light flashes or spots
  • Pain or redness of the eye

Anyone with a family history of glaucoma is recommended to have routine glaucoma screening from the age of 40.

If you have diabetes, you should have an annual eye screening to check for diabetic retinopathy, a condition which can lead to the loss of vision if untreated.

The centre has consultation rooms, procedure rooms and a laboratory for making custom ocular prostheses (artificial eyes) as well as a children’s play area for the convenience of families with young children.

About Infertility

Infertility is defined as a couple not able to conceive after 12 months of regular unprotected sexual intercourse. Infertility affects 1 in every 7 couples. This makes it as one of the most common problem faced for people between the ages of 20 and 45.

In addition, the longer a woman tries to get pregnant without conceiving, the lower is her chances to get pregnant without medical treatment. Most (85%) couples with normal fertility will conceive within a year of trying. If a couple doesn’t conceive in the first year, their chances of conceiving gets lower each month. The good news for couples struggling to conceive is that most people are now able to achieve their dream of starting a family through the huge advances made in fertility treatment.

Treatment begins with understanding the cause of any fertility issues, through fertility assessments for both partners. Basic investigation and conventional treatment modalities are usually advised prior to advanced methods used to attempting pregnancy via artificially or partially artificial methods called Assisted Reproductive Techniques (ART). These include the following:

Semen Analysis

A common test for males, the semen analysis evaluates certain characteristics of the semen and determines the number and quality of the sperm.

Sperm Freezing / Banking

It is a procedure to preserve the sperm cells for future use. Semen can be used successfully after cryopreservation. It is advisable as a ‘back up’ semen sample for specific situations such as difficulty in producing a sample on the day, male partner away for employment, or declining semen quality. Sperm freezing is also available for men before they go through chemo or radiotherapy and before a vasectomy and prostate surgery.

Surgical Sperm Retrieval

This treatment is suitable for:

  • An obstruction preventing sperm release, due to injury or infection
  • Congenital absence of the vas deferens (men born without the tube that drains the sperm from the testicle)
  • Vasectomy
  • Non-obstructive azoospermia – the testicles are producing such low numbers of sperm that they don’t reach the vas deferens.

Timed Intercourse

It is a natural family planning method to conceive based on the right timing for intercourse during the fertile window either using the natural cycle or with medications.

IUI is a simple procedure where the doctor uses a small catheter to put the washed sperm into the uterus after minimal stimulation with medications or injections.

Oocyte Freezing / Banking

Oocyte cryopreservation is aimed at three particular groups of women:

  • Those diagnosed with cancer who have not yet begun chemotherapy or radiotherapy
  • Women with a family history of early menopause
  • Those who would like to preserve their future ability to have children, either because they do not yet have a partner, or because of work commitment

Embryo Biopsy

Woman of an advanced age (>35 years old) is at an increased risk for chromosomal abnormalities and embryo biopsy can be done to rule out the chromosomal abnormalities and genetic diseases in pregnancies. The biopsy is performed by taking a small sample of cells from the embryo using a micromanipulator and micropipette and the cells will be sent out for genetic screening.

In Vitro Fertilization (IVF)

IVF is the most common kind of ART. It is done by joining the sperm and eggs outside a woman’s body in a small dish in the laboratory to allow fertilisation to occur. One or more embryos are then transferred into the woman’s uterus. Patient will need to undergo an ovulation induction with a combination of a few drugs to stimulate the ovaries to produce more follicles or eggs.

Intra Cytoplasmic Sperm Injection (ICSI)

ICSI is a procedure whereby a single healthy sperm is injected into the centre of the egg using a delicate microneedle. This method is used when the sperm have difficulty penetrating the egg or when the sperm count is very low. The wife will still need to undergo ovulation induction with a combination of a few drugs to stimulate the ovaries to produce more follicles or eggs.

Embryo Freezing / Banking

Embryo freezing is useful for storing surplus embryos after a cycle of IVF, as patients who fail to conceive may become pregnant using such embryos without having to go through a full IVF cycle again. Or, if pregnancy occurred, they could return later for another pregnancy. Embryo freezing is also an alternative to couples that might not want to transfer too many embryos and risk a multiple pregnancy.

Blastocyst Transfer

It is an extended embryo culturing to provide high quality embryos which are capable of continued development and result in higher live birth rate. This is particularly useful for younger women with good prognosis for pregnancy from IVF. However, only less than 55% of the human embryos progress to the blastocyst stage after 5 days of culture and there will be fewer embryos for freezing.

Frozen Embryo Transfer (FET)

A FET cycle is for women with wish to use their frozen embryos in the future, e.g. after delivery of a child, after an unsuccessful stimulated IVF cycle. FET is also a good option when the endometrium lining or hormone is showing signs of not suitable for a fresh embryo transfer during a stimulated IVF cycle.

Laser Assisted Hatching (LAH)

It is a laboratory procedure performed mostly on cleavage stage embryos before being transferred back to the patient’s womb. During the procedure, a small opening is made on the outer shell of the embryo. LAH can assist the embryo in breaking out of this shell to facilitate the implantation process.

Another infertility treatment which is a component of assisted conception is Intrauterine Insemination (IUI)

Hand & Microsurgery

We use our hands for everyday tasks and at some point or other, we may have experienced a minor injury like pain or swelling to our hand, wrist or even finger that heal on their own. However, sometimes some more serious injuries occur as a result of falls or recreational activities (acute injuries) or even the everyday wear and tear or the overuse of certain muscles (overuse injuries).

Acute injuries include bruises, strains, sprains, fractures or injury to the ligaments, tendons or muscles. They occur as a result of a direct impact, a fall, or a twisting, bending or jamming of a limb.

In contrast, overuse injuries happens when repeated activity or too much stress is placed on a joint or tissue when carrying out an activity. Common overuse injuries include carpal tunnel syndrome, tendinosis and de Quervain’s disease.

Treatment for a finger, hand, or wrist injury includes first-aid, medication, application of a splint or cast, physical therapy, and in some cases, surgery. Treatment depends on the location, type, and severity of the injury. Your doctor will also need to assess how long ago the injury occurred and your age, health condition, and lifestyle (how active you are at work and recreational pursuits).

The Hand and Microsurgery Centre manages all traumas and diseases affecting the hand, wrist, or upper limb that affect hand function and where needed, microsurgical reconstruction is performed. The range of services includes:

The Tumour Board is the name of Sunway Cancer Centre’s weekly patient-centric multidisciplinary conference meeting, gathering healthcare professionals from multiple specialties, including surgeons, physicians, oncologists, radiologists, and pathologists, working as a streamlined team to investigate complex cancer cases and to review the pros and cons of various treatment options, from acute to long-term.

The doctor will evaluate if the cancer case is appropriate for a tumour board review.

By the end of the meeting, the team collectively decides the most ideal treatment plan for the individual patient, with the sole aim of achieving the best possible outcomes.

1 in 10 adults worldwide have Chronic Kidney Disease (CKD), a condition where the kidney slowly and progressively loses function over time. Medical Expulsive Therapy

Medication is given with the intent to spontaneously pass suitably sized ureteral stone.

Ureteroscopy / Flexible Uretero-Renoscopy (URS / F-URS)

A very thin tube (ureteroscope) is passed into the urinary tract to the stone’s location, where instruments, such as a stone basket or Holmium laser, can then be used to remove the stone or break it up for easier removal.

Shock Wave Lithotripsy (ESWL)

This commonly used procedure uses shock waves to break up a kidney stone. It is the least invasive surgical technique and is generally performed as a daycare procedure.

Percutaneous Nephrolithotomy (PCNL)

This surgical technique is used for the largest types of stones. It generally requires a small (1 cm) incision at the side or back and the use of an endoscope to remove the kidney stone.

Important notes for preparation before appointment:

  • Please fast for at least 8 – 10 hours prior to the appointment. You may drink plain water only.
  • Postpone taking any routine morning medication (except Hypertension drugs), unless advised by your doctor, until after the blood investigation.
  • Ladies who are going for mammogram are to avoid using deodorant or talcum powder.
  • Ladies going for mammogram are to bring their old films for comparison.
  • Pap Smear should best be done 7 days after the last day of your menstruation.

Things to bring:

  • Comfortable clothing and shoes for your Cardiac Stress Test (Treadmill).
  • Previous medical records, if any, especially X-rays, MRI or CT Scan from your last check-up for comparison.
  • Bring your glasses (spectacles) for your vision test.

How long will the screening take?

The duration of each health screening package varies. On average, the entire health screening process including medical examination by a doctor will take between 2 – 4 hours depending on the health screening package selected.
